Newly updated Disabled Parking Permit Handicap Form

It has been brought to our attention that customers have been coming in to our agencies with out of date Disabled Parking Permit forms. Unfortunately when a patient comes into the agency with an out of date form we cannot process their Handicap Parking Permit Request and they must get a new form completed. We are sending this message out as a friendly reminder that the Application for Disabled Parking Permit form was revised on 6/22/22. Below is a link to the most up to date application for your offices to use for all Disabled Parking permits.
